You have seen it and maybe resented it. Two people eat about the same amount, and one stays lean while the other gains, or one is visibly muscular and the other soft, on what looks like the same food. It is tempting to conclude that calories are a myth. They are not. Energy balance still governs body weight. What varies between two people is everything around that equation: how much energy their bodies burn without trying, what their calories are made of, and what their bodies are built from. Once you see those levers, the mystery mostly disappears.

Calories still count, but the burn side varies
Start with what is true: to gain fat you need to eat more energy than you burn, and to lose it you need the reverse. That has not been repealed. The trap is assuming that eating the same means burning the same. It does not. The energy you burn in a day has several parts, and two of them vary enormously from person to person. Your resting metabolism, the energy to keep you alive at rest, differs with body size and muscle. And the energy you spend moving, especially the unplanned movement you never think about, can differ by hundreds of calories between two similar people.
So when two people eat identical calories and end up different, it is usually not because calories failed. It is because their total burn was never actually equal. One of them was quietly spending more, and the difference compounds over months into visibly different bodies.
Energy balance is real. What varies is the balance point, because two people eating the same rarely burn the same. The equation is honest, the inputs just differ.
The biggest hidden lever: NEAT
The single most striking explanation comes from a classic overfeeding study. Researchers fed 16 people 1,000 calories a day above what they needed for eight weeks and measured where the extra energy went. Fat gain varied tenfold between individuals on the exact same surplus. The thing that predicted who stayed lean was not willpower or metabolism in the usual sense. It was non-exercise activity thermogenesis, or NEAT: the energy burned through fidgeting, standing, pacing, gesturing, and generally not sitting still. Some people unconsciously ramped up their movement when overfed and burned off most of the surplus. Others did not, and stored it.
NEAT is invisible and largely automatic, which is why it feels unfair. The lean friend who eats a lot may simply move more all day without noticing, taking the stairs, tapping their foot, rarely sitting for long. Over a year, a few hundred calories a day of extra fidgeting is the difference between staying lean and slowly gaining. You can nudge your own NEAT up by walking more and sitting less, which is one of the most underrated tools in body composition.
What your calories are made of changes the maths
Not all calories cost the same to process. Your body spends energy digesting and storing food, an effect called the thermic effect of food. It is not equal across the macronutrients. Protein has by far the highest cost, with roughly 20 to 30 percent of its calories burned just in processing it. Carbohydrate sits around 5 to 10 percent, and fat is lowest at close to 0 to 3 percent.
Play that out. Two people eat 2,000 calories, but one gets a large share from protein and the other from fat and refined carbs. The higher-protein eater burns noticeably more of their intake just digesting it, and tends to feel fuller so they are less likely to overeat later. The net calories they keep are lower even though the plate said the same number. This is one honest reason a protein-forward diet helps body composition, and it applies whether your protein comes from animal foods or from beans, lentils, tofu, tempeh, and other plant sources.

A gram of protein and a gram of fat are not equal once your body processes them. Building meals around protein means you keep fewer of the calories you eat and feel fuller doing it.
Same weight, different bodies: muscle versus fat
Now the other half of looking different: two people can weigh the same and eat the same and still look nothing alike, because weight says nothing about what the body is made of. A kilogram of muscle is denser and takes up far less space than a kilogram of fat, so the person carrying more muscle and less fat looks leaner and more defined at the identical scale weight. This is body composition, and it is built by training, not by diet alone.
The lever here is resistance training paired with enough protein. A large meta-analysis found that protein supports muscle gain from training up to around 1.6 grams per kilogram of body weight per day, beyond which extra protein adds little. So of two people eating the same calories, the one who lifts and eats adequate protein steadily converts a similar intake into a firmer, leaner-looking body, while the one who does neither does not. Same calories in, very different structure to show for it.
This is why the scale is a poor judge of progress on its own. You can hold your weight and completely change how you look by trading fat for muscle over months. Two people at 70 kilograms can look like they belong to different sports.
Day-to-day noise that fools the mirror
Finally, some of the difference you see is temporary and has nothing to do with fat. Your body holds water for many reasons: how many carbs you ate recently, salt intake, hormones, stress, sleep, and how much food is simply moving through your gut. Carbohydrate in particular is stored with water, so a high-carb day can add a kilogram or more on the scale and a softer look overnight, then vanish just as fast. None of that is fat gain.
So two people photographed on the same day might differ partly because one is holding more water and food than the other in that moment. This is why single weigh-ins and single mirror checks mislead. Judge body composition by the trend over weeks, using the same conditions, rather than by any one day. The fat that matters moves slowly. Almost everything that moves fast is water and timing.
A 1 to 3 kilogram swing across a day or two is water, glycogen, and gut contents, not fat. Comparing yourself to someone else on a single day compares two piles of noise.
