A shoulder that catches or aches every time you press overhead is one of the most common complaints in the gym. The instinct is to grind through it or to stop pressing altogether. Neither is the answer. In most people, a nagging pressing shoulder comes down to two fixable things: not quite enough range to reach overhead cleanly, and not enough control of the shoulder blade to keep the joint moving well through that range. Get both back and the press usually stops complaining. Here are the drills that do it, plus the honest caveat about when a shoulder needs a professional, not a foam roller.

Why the shoulder pinches when you press
To press a weight straight overhead, your arm bone has to rotate up in its socket while your shoulder blade rotates upward underneath it, tilting the socket toward the ceiling so the arm has somewhere to go. When that coordination is off, the top of the arm bone and the soft tissue around it can crowd the narrow space beneath the tip of the shoulder blade. That crowding is what many people feel as a pinch at the top of a press. Clinicians call the broad picture subacromial pain.
The important thing is what usually drives it. For most gym-goers with a healthy shoulder, the pinch is not a torn structure. It is a movement problem: the shoulder blade does not rotate up well, the upper back is too stiff to help, or the arm simply lacks the last bit of overhead range, so the joint travels through a cramped position rep after rep. Those are trainable. That is why the same shoulder that pinches under a strict barbell press today can press cleanly a few weeks later with no change except better movement.
Two things have to be true to press pain free. You need the range to reach overhead, and you need the control to keep the shoulder blade moving well through that range. Most nagging shoulders are short on one or both. The drills below target each.
What the research says about fixing it
Exercise is the first-line answer for this kind of shoulder pain, and the evidence is reassuringly boring. Systematic reviews and meta-analyses of people with subacromial and rotator-cuff-related shoulder pain find that structured exercise, especially work that targets the rotator cuff and the muscles that control the shoulder blade, reduces pain and improves function. The gains are moderate rather than magic, but they are real, and exercise performs as well as many other treatments without the downsides.
There is a useful nuance in that research. Scapular and cuff exercise reliably improves pain and function, but it does not always increase raw range of motion by much. That fits the two-part picture: often the range is roughly there, and what changes the pinch is teaching the shoulder blade and cuff to control the joint through the range you already have. So a good plan does both, restores any missing range and drills the control.
Check your overhead shoulder
Two quick checks tell you whether range, control, or something that needs a professional is behind your pinch.
- The wall overhead reach. Stand with your back to a wall, lower back flat against it and ribs down. Raise both arms overhead and try to touch the wall without your lower back arching away. If you cannot reach cleanly, you are short on overhead range, often from a stiff upper back as much as the shoulder itself.
- The slow controlled press. Press a light weight or empty bar overhead slowly and watch. A shoulder that shrugs up hard early, or an arm that drifts forward, points to weak shoulder-blade control rather than a pure range problem.
- The pain screen. Note the pain itself. A mild, familiar pinch at the very top that eases as you warm up is usually a movement issue. Sharp pain, pain at rest or at night, a feeling of instability, or clear weakness is different, and a reason to get it looked at.
A clean overhead reach against the wall with ribs down, and a press where the shoulder blade sets down and rotates smoothly rather than shrugging up in a panic. Get those two and the pinch usually fades.
The drills that let you press pain free
These target both halves of the problem: the range to reach overhead and the control of the shoulder blade and cuff through it. Do the mobility pieces most days and the control pieces two or three times a week. Keep everything pain free, working up to the edge of a stretch, never into a sharp pinch.
- Wall slides. Back to a wall, forearms on it, slide them up overhead keeping your lower back flat and ribs down, then back. This trains overhead range and the upward rotation of the shoulder blade together, in the exact pattern the press uses. Do 10 to 12 slow reps.
- Band pull-aparts. Hold a light band at shoulder height, arms straight, and pull it apart by squeezing your shoulder blades, then return slowly. This strengthens the mid-back and the muscles that set the shoulder blade, the control half of the equation. Do 15 to 20 reps.
- Prone or wall external rotations. With a light band or dumbbell, keep your elbow bent at 90 degrees and rotate your forearm outward against resistance, then back slowly. This targets the rotator cuff, the small muscles that centre the arm in the socket so it does not crowd the top. Do 12 to 15 reps per side, light.
- Sleeper-friendly cross-body stretch. Gently draw one arm across your body with the other, feeling a stretch at the back of the shoulder. Tightness there can nudge the arm forward in the socket. Hold 20 to 30 seconds per side, easy, never forcing into pain.
These drills suit the common movement-driven pressing pinch. They are not treatment for an injury. Sharp pain, night pain, a shoulder that feels unstable, or clear weakness is a reason to see a doctor or physiotherapist before you keep loading it. Pain that gets worse with the drills is a stop sign.
How to warm up so the press feels good
Timing matters. Right before you press, you want to raise the temperature and prime the movement, not do long static stretches. Research on stretching shows that long static holds right before a strength effort can briefly reduce power, so save those for later and use dynamic drills to warm up.

If a strict barbell press still pinches after a proper warm-up, change the tool rather than fight it. A neutral-grip dumbbell press or a landmine press lets most people train the overhead pattern in a friendlier path for the shoulder while the drills do their work. Progress back to the barbell when it feels clean.
