You get a blood test back, find the vitamin D line, and see a number next to it. Whether that number looks reassuring or alarming depends on two things most people never notice: which expert cut-off your lab decided to use, and which units it reported in. The same blood sample can read as 30 in one country and 75 in another, and can be called perfectly fine by one guideline and mildly low by another. None of that is a lab error. It is a genuine disagreement about where the line should sit. Once you understand it, your result stops being a source of worry and becomes a simple, useful piece of information.
The short version
- The test measures 25-hydroxyvitamin D, written 25(OH)D. It is the storage form and the standard marker of your vitamin D status.
- Labs report in either **nanograms per millilitre (ng/mL)** or **nanomoles per litre (nmol/L)**, and 1 ng/mL equals 2.5 nmol/L, so always check the unit before you judge the number.
- The public-health floor for adequacy is about 20 ng/mL (50 nmol/L). A separate clinical guideline preferred above 30 ng/mL (75 nmol/L), which is why reports disagree.
- Deficiency generally means below about 12 ng/mL (30 nmol/L), the level linked to bone problems, and is the number worth acting on.
- The current advice is to not routinely test most healthy people. Testing makes sense when there is a real reason, and results should be read with your doctor.
What the test actually measures
When a lab reports your vitamin D, it is almost always measuring 25-hydroxyvitamin D, shortened to 25(OH)D. This is the form your liver makes after you get vitamin D from sun, food, or a supplement, and it circulates in your blood for weeks. Because it is stable and reflects your recent intake and sun exposure, it is the agreed-upon marker of vitamin D status worldwide.
There is a second, more active form called 1,25-dihydroxyvitamin D, but it is tightly controlled by your body and can look normal even when your stores are low. So it is not the number to check for ordinary status. Unless a specialist has a specific reason to order it, the line you care about is 25(OH)D.
On your report, look for 25-hydroxyvitamin D or 25(OH)D. That is your vitamin D status. Everything below is about reading that single value correctly.
First, check the units
Before you decide whether a number is good or bad, find the unit printed next to it. There are two in common use, and they produce wildly different figures from the same blood. The United States and a few other places use nanograms per millilitre (ng/mL). Most of the rest of the world uses nanomoles per litre (nmol/L).
The conversion is simple: **1 ng/mL equals 2.5 nmol/L**. So a result of 30 ng/mL is the same as 75 nmol/L. If you compare a friend's number, or an old result, or a figure you read online, without matching the units first, you can scare yourself over nothing or feel safe when you should not.

Multiply a ng/mL result by 2.5 to get nmol/L, or divide a nmol/L result by 2.5 to get ng/mL. Getting the unit wrong is the single most common reason people misread this test.
Why two guidelines give two verdicts
Here is the part that trips people up. Two respected bodies drew the line in two different places, and both numbers are still floating around lab reports and articles today.
The Institute of Medicine, which sets nutrient intakes for the general population, concluded that a level of about 20 ng/mL (50 nmol/L) meets the needs of nearly everyone for bone health. It treats this as a population floor, not a target to beat. It also identified true deficiency, the level tied to soft, weakened bones, as below roughly 12 ng/mL (30 nmol/L).
The Endocrine Society, in a 2011 clinical guideline aimed at patients with reasons to worry about their bones, preferred a higher bar of above 30 ng/mL (75 nmol/L), calling 21 to 29 ng/mL insufficient. That is a stricter, more cautious clinical target for at-risk people, not a claim that everyone below 30 is unwell. Both groups looked at largely the same evidence and made a reasonable judgement call in different directions. That is the whole source of the disagreement.
Should you even be testing?
It is worth stepping back, because the newest expert advice pushes against routine testing. In 2024 the Endocrine Society updated its stance and recommended not screening healthy people for vitamin D, at any age, because for most people a normal or low number does not change what they should sensibly do. Testing everyone tends to generate anxiety and follow-up costs without improving health.
Testing earns its place when there is a real reason. Symptoms of deficiency such as bone pain or muscle weakness, a condition that affects how you absorb fat, certain medications, very limited sun exposure, or a clinician following up on low bone density are all sound reasons. If your test was ordered as part of a health check with no particular concern, the result is still interesting, it just should not send you down a rabbit hole.
Many people can cover their needs with the daily intake most guidelines suggest, around 600 to 800 international units (IU) of vitamin D, without ever testing. A supplement at that dose is low risk. Talk to your doctor before taking high doses or if you have kidney, liver, or calcium problems.
Reading your own result, step by step
Put it together and reading your report becomes a short, calm routine rather than a scramble to compare numbers online.
- Confirm it is 25(OH)D. Make sure the line you are reading is 25-hydroxyvitamin D and not the active 1,25 form, which is a different test.
- Read the unit. Note whether it says ng/mL or nmol/L, and convert if you need to, using the 2.5 factor, before you compare it to anything.
- Find deficiency first. If you are below about 12 ng/mL (30 nmol/L), that is the level that genuinely matters and is worth acting on with your doctor.
- Place yourself against both cut-offs. Above 20 ng/mL you meet the population floor. Above 30 ng/mL you also clear the stricter clinical bar. Both are reasonable frames.
- Do not chase a high number. There is no added benefit to pushing far above 30 ng/mL, and very high levels from over-supplementing can cause harm.
What to do with a low or borderline result
If your number is genuinely low, the fix is usually straightforward and the response should be proportionate. A clear deficiency is treated with vitamin D under medical guidance, sometimes at a higher dose for a short period to rebuild stores, then a maintenance dose. A borderline result in an otherwise well person often just means a modest daily supplement, especially through darker winter months at higher latitudes.
You can also support your level through food and sun without megadoses. Oily fish such as salmon, sardines, and mackerel carry vitamin D, as do egg yolks. If you eat plant based, reliable sources include UV-exposed mushrooms and foods fortified with vitamin D, such as many plant milks, some cereals, and certain spreads, so check the label for an added-vitamin-D line. Vitamin D2 and the D3 made from lichen are both suitable for a vegan diet and both raise blood levels. A little regular midday sun on skin helps in the warmer months, balanced against skin-cancer sense.
More is not better. Routinely taking very high doses can push blood calcium up and cause real harm. Above the everyday supplement range, dosing should be guided by your doctor and rechecked, not self-prescribed indefinitely.
