A vegan diet can be complete, but three nutrients do more damage when neglected and take more thought to cover than any others. Vitamin B12, iron, and the long-chain omega-3 fats EPA and DHA are the big three. Two of them, B12 and long-chain omega-3, are genuinely hard to get from unsupplemented plants. The third, iron, is present in plants but absorbed less efficiently. Get these three right and you have handled most of what makes vegan nutrition different from any other diet. This is the detailed version: what each one does, how to cover it, and how to confirm you actually have.
The short version
- **B12** must come from a supplement or fortified foods on a vegan diet, full stop. It is not reliably present in plant foods.

Vitamin B12: supplement, without exception
B12 is the clearest-cut of the three. It is produced by bacteria, not by plants or animals directly, and in the modern food chain there is no reliable plant source. A review of the evidence found vegans have markedly higher rates of B12 depletion and deficiency than vegetarians or meat-eaters, in every age group examined. A long-term shortfall can cause a form of anaemia and nerve damage, and some of the nerve damage may not fully reverse. This is why every serious vegan guideline says the same thing: supplement it.
The practical part is easy. B12 supplements come as cyanocobalamin, the cheap and stable form used in most studies, and methylcobalamin. Because absorption of a single large dose is inefficient, a common approach is either a smaller daily dose or a larger dose once or twice a week. Fortified foods such as some plant milks, nutritional yeast, and breakfast cereals also count, provided you eat them reliably. Claims that spirulina, unwashed produce, or fermented foods supply B12 do not hold up, they often contain inactive look-alikes that do not do the job.

A vegan diet with no B12 supplement and no fortified foods is the one version that is genuinely unsafe long term. Do not rely on algae, fermented foods, or unwashed vegetables. Use a real supplement or properly fortified foods, and check your level with a doctor if unsure.
Iron: present in plants, absorbed less easily
Iron is a different problem from B12. Plants contain plenty of it, but in the non-heme form, which the gut absorbs less efficiently than the heme iron in meat. On top of that, phytates in whole grains and pulses bind iron and lower absorption further. Reviews estimate that overall iron absorption from a vegetarian or vegan diet may be roughly half that from a mixed diet, which is why some guidance suggests plant-based eaters aim for a higher iron intake.
You have real levers here. Vitamin C eaten in the same meal as iron-rich plants sharply increases how much iron you absorb, by keeping the iron in a form the gut takes up more readily and by counteracting phytates. Tea and coffee, by contrast, contain tannins that block iron, so keeping them away from your main iron meals helps. Soaking, sprouting, and fermenting grains and pulses lowers their phytate and frees more iron. Good plant sources include lentils, beans, tofu, tempeh, wholegrains, pumpkin seeds, and dark leafy greens.
- Build meals around iron-rich plants. Lentils, beans, tofu, wholegrains, seeds, and dark greens should appear most days.
- Add vitamin C to those meals. Peppers, tomatoes, citrus, or fruit alongside the meal can multiply how much iron you absorb.
- Move tea and coffee away from meals. Their tannins block iron, so enjoy them between meals rather than with your main iron sources.
- Prep pulses and grains well. Soaking, sprouting, and fermenting cut phytate and release more of the iron and zinc inside.
People who menstruate have higher iron needs and a greater risk of running low. If you are vegan and often tired, breathless, or pale, ask your doctor to check your ferritin rather than self-prescribing iron pills, which can cause problems if taken when not needed.
Omega-3: go straight to algae
The omega-3 story trips people up because there are two different things sharing the name. The short-chain form, ALA, is abundant in flax, chia, walnuts, and rapeseed oil, and vegans usually get plenty. The long-chain forms, EPA and DHA, are the ones tied to heart and brain health, and they normally come from oily fish. Your body can convert ALA into EPA and DHA, but the conversion is limited: the step to EPA is modest and the step to DHA is often very low, frequently under a few percent.
That means eating flax and chia is good, but leaning on conversion alone is a shaky way to secure the long-chain fats. The direct fix is an algae oil supplement. Algae is where fish get their EPA and DHA in the first place, so an algae supplement gives vegans the same fats without the fish and without depending on conversion. A daily ALA food plus a small algae oil supplement is the complete, no-fish solution.
Eat ALA from flax, chia, and walnuts for the short-chain form, and take an algae oil supplement for the long-chain EPA and DHA that plant conversion cannot reliably deliver.
How to know your plan is working
The reassuring part of the big three is that you do not have to guess. A simple blood test can confirm whether your habits are doing the job, which turns vague worry into a number you can act on.

For most healthy vegans, a check every year or two is plenty once a good routine is in place. Test sooner if you feel persistently tired, notice tingling or numbness, or are pregnant, breastfeeding, or raising a child on a vegan diet, since needs are higher and the stakes are greater at those times. Any low result should be discussed with a doctor or dietitian rather than fixed by throwing supplements at it, because the right dose and form depend on how low you are and why.
Do not diagnose yourself from how you feel. Fatigue has a hundred causes. A blood test for B12 and ferritin turns guesswork into facts, and it is the single most useful thing a long-term vegan can do to stay safe.
