Most supplement debates are about squeezing out a little extra. B12 is different. It is the one nutrient a well-planned vegan diet still cannot supply on its own, because it does not come from plants at all. The good news is that fixing this is cheap, easy, and completely settled. The catch is that deficiency builds slowly and quietly, so people who feel fine assume they are fine, right up until they are not. If you eat no animal foods, a reliable B12 source is not optional. It is the price of doing plant-based well.

Why plants cannot supply B12
Vitamin B12, also called cobalamin, is not made by plants or by animals. It is made by bacteria. Grazing animals pick it up from microbes in their gut and in soil, then concentrate it in their tissues, which is why meat, fish, eggs, and dairy carry it. Plants have no need for B12 and no way to make it, so fruit, vegetables, grains, beans, nuts, and seeds contain essentially none you can rely on.
You may have read that certain plant foods contain B12, such as some mushrooms, seaweed, or unwashed produce. The problem is twofold. The amounts are tiny and wildly inconsistent, and some seaweeds contain B12 analogues, molecules that look like B12 to a lab test but do not work in your body and may even block the real thing. The Academy of Nutrition and Dietetics, in its position on vegetarian and vegan diets, is blunt about this: plant-based eaters need a reliable source from fortified foods or supplements, not from these unreliable options.
A well-planned vegan diet covers protein, iron, calcium, and the rest perfectly well with ordinary food. B12 is simply the one exception where biology, not planning, forces you to add a source. Cover it and the diet stands up to any other.
What running low actually does
B12 does two jobs you cannot skip: it helps build healthy red blood cells and it keeps the protective sheath around your nerves intact. When it runs low, both start to fail. In the blood you get a specific kind of anaemia where red cells grow large and inefficient, leaving you tired, breathless, and foggy. In the nervous system you get tingling or numbness in the hands and feet, balance problems, and memory or mood changes.
The reason B12 deficiency is dangerous is that it is patient. Your liver stores a reserve that can last months to years, so someone who stops getting B12 often feels normal for a long time while the tank quietly drains. By the time symptoms are obvious, damage may already be under way. The most important thing to know is this: some of the nerve damage from prolonged B12 deficiency does not fully reverse even after you start supplementing, which is exactly why prevention beats waiting for symptoms.
A diet rich in leafy greens and beans is high in folate, which can hide the anaemia caused by low B12 on a basic blood count while the nerve damage keeps progressing underneath. A plant-based eater can therefore look fine on a simple test and still be heading for trouble. Do not rely on feeling well as proof.
How much you need, and why the dose looks odd
The amount your body actually uses is tiny. The adult recommended dietary allowance is 2.4 micrograms a day, rising slightly in pregnancy and breastfeeding. So why do vegan supplements often say 500, 1000, or even 2000 micrograms? Because of how you absorb it.
B12 is absorbed through a special escort protein in your gut called intrinsic factor, and that system can only handle roughly 1.5 micrograms at a time before it is saturated. Above that, only about 1 percent of a dose leaks across the gut wall by simple diffusion. That is why a large occasional dose still works: 1 percent of 2000 micrograms is 20 micrograms, comfortably more than you need. It also means smaller, more frequent doses are absorbed far more efficiently per microgram, so you have two sensible strategies.

Fortified foods, supplements, or both
You do not strictly need a pill if you are diligent with fortified foods, and many plant-based eaters use a mix. Common fortified sources include plant milks, some breakfast cereals, and nutritional yeast, the flaky, savoury, cheesy-tasting product that is a genuine plant source once it has been grown on a B12-enriched medium. Always check the label, because fortification is not guaranteed and amounts vary between brands and countries.
- Fortified plant milk: a typical glass supplies a useful fraction of a day's B12, but only if the carton actually lists it.
- Fortified nutritional yeast: a savoury topping for pasta, popcorn, or scrambled tofu that can carry a meaningful dose per tablespoon when fortified.
- Fortified breakfast cereals: convenient, though often paired with a lot of added sugar, so read the panel.
- A dedicated supplement: the most reliable option, unaffected by whether you happened to buy the fortified brand this week.
Take one cheap B12 supplement on your chosen schedule and treat any fortified foods as a bonus rather than your main plan. That way a change of brand or a rushed week never quietly puts you at risk. It is the lowest-effort way to make sure the one non-negotiable nutrient is genuinely covered.
Should you get tested?
If you have supplemented consistently for years, a test is reassurance rather than a necessity. Testing earns its place if you have been plant-based without a reliable source for a while, if you are pregnant or planning to be, or if you have any of the warning signs such as persistent fatigue, tingling, or numbness. A standard serum B12 test is a starting point, but because it can be thrown off by inactive analogues, a more informative picture comes from markers such as methylmalonic acid or active B12 (holotranscobalamin). These were the more accurate measures used in the research on how common deficiency really is.
- Start a reliable source today. Do not wait for a test result. Beginning a supplement now costs almost nothing and stops the tank from draining further.
- Ask about the better markers. If you test, mention methylmalonic acid or active B12 to your doctor, especially if a basic serum result looks borderline.
- Flag symptoms early. Persistent tingling, numbness, or unexplained fatigue deserve a conversation with a clinician rather than a wait-and-see approach, because nerve damage can be lasting.
