Most supplements live in a fog of weak studies and loud marketing. Creatine is the rare exception. It has hundreds of trials behind it and a formal position stand from the International Society of Sports Nutrition. That lets us do something unusual: separate what is genuinely proven from what is hopeful, and from what is myth.

- It works by refilling your muscles' fast energy system, letting you do a rep or two more across a session, which adds up over months.

What creatine actually is
Creatine is not a drug and not a stimulant. It is a compound your own body already makes, and that you already eat, mostly in meat and fish. Your liver produces some, and roughly 95 percent of what you carry sits in your muscles, ready for use.
Its job is energy. For very short, very hard efforts, a heavy set, a sprint, a jump, your muscles run on a fast fuel system built around a molecule called phosphocreatine. That system empties in seconds. Supplementing with creatine tops up the tank, so you can produce a little more force and recover that fuel a little faster between efforts.
Creatine does not build muscle directly. It lets you train slightly harder, and it is that extra work, repeated across weeks and months, that builds the muscle. It is a training amplifier, not magic powder.
What the evidence clearly shows
This is the part that is genuinely settled. The International Society of Sports Nutrition, after reviewing the whole body of research, called creatine monohydrate the most effective supplement available for increasing high intensity exercise capacity and lean body mass during training. That is a strong statement, and the evidence backs it.
- More work per session. By refilling the fast energy system, creatine lets you squeeze out an extra rep or hold power for slightly longer in short, hard efforts.
- More lean mass over time. Part of the early gain is water drawn into the muscle, but across months the added training volume drives real muscle growth.
- Faster recovery between hard sets, which matters most in strength, sprint, and jump style training.
- It helps most where the effort is short and intense. It does little for steady, long distance endurance.
One honest note on the scale of the effect. Creatine is not a transformation. It is a modest, reliable edge. If your training and eating are a mess, creatine will not rescue them. If they are dialled in, it gives you a real few percent more to work with.
How to take it, without overcomplicating
There is a lot of noise here, and almost none of it matters. The goal is simply to fill your muscle stores and keep them full. There are two ways to get there.

Creatine monohydrate. It is the form used in almost every study, it is the cheapest, and there is no good evidence that pricier forms (hydrochloride, buffered, liquid) work any better. Fancy versions mostly buy you a nicer label.
If you eat little or no meat, this matters more for you, not less. Vegetarians and vegans tend to start with lower muscle creatine stores, which is likely why some studies find they respond more strongly to supplementing. Plant based diets simply provide less creatine to begin with, so topping it up can make a clearer difference.
Is it safe? The kidney question, honestly
The most common worry is that creatine damages your kidneys. This fear comes partly from a lab test: your kidneys are often checked using a marker called creatinine, and taking creatine can nudge that marker up without anything actually being wrong. A higher reading looks alarming but does not mean harm.
In people with healthy kidneys, decades of research and the ISSN position stand find no evidence that standard creatine doses cause kidney damage. Studies have run for months and years at these doses without renal harm in healthy people.
You have existing kidney disease, reduced kidney function, or take medication that affects your kidneys. Creatine is cleared by the kidneys, so it is not the group to self experiment in. Anyone with a diagnosed condition, or who is pregnant, should check with a doctor before starting a supplement.
The other common complaints are minor: some people get mild stomach upset with large single doses, which usually goes away by splitting the dose or taking it with food. Some hold a little extra water early on. Neither is dangerous.
The myths, and the maybes
Myth: creatine causes hair loss
This one traces back to a single small study that measured a hormone linked to hair loss, not hair loss itself. Later research has not shown that creatine actually makes people lose hair. As it stands, the hair loss claim is not supported by good evidence.
Myth: it is a steroid, or it makes you fat
Creatine is not a hormone and not a steroid. Any quick early weight gain is mostly water pulled into the muscle, not fat. It does not bloat your face or make you gain fat.
Maybe: the brain benefits
This is the honest grey area. Your brain also uses creatine, and there is real interest in whether supplementing helps thinking, memory, and resilience to stress. The current picture is mixed. The clearest signals show up in specific situations: people who are sleep deprived, and vegetarians who start low. For a well rested meat eater, do not expect a noticeable mental boost yet. It is a promising area, not a settled one.
Proven: modest gains in strength, power, and lean mass, and safety in healthy people. Promising but unsettled: brain and cognition benefits. Not supported: hair loss, bloating, kidney harm in healthy people. Buy the plain monohydrate and skip the hype.
