The wellness industry sells hundreds of tests, and most of them tell you very little about how long you will live. The measures that genuinely predict lifespan are a surprisingly short list, and the strongest ones are not exotic. They are your fitness, your strength, and how fast you can walk. Some cost nothing to check. Here is what the long-run studies actually found, and how to read your own numbers without spiralling over every decimal point.
The short version
- Cardiorespiratory fitness, measured as **VO2 max**, is the single strongest predictor of how long you live, and in one study of **122,000 people** there was no upper limit to the benefit.
- Grip strength is a cheap, fast stand-in for whole-body strength. Each 5 kilogram drop tracked with about a 16 percent higher risk of dying from any cause.
- Walking speed predicts survival in older adults so well that it is sometimes called the sixth vital sign.
- The blood markers worth watching are few: markers of blood sugar, blood pressure, blood lipids, and inflammation. Most of the panel you can buy adds little.
- The good news is that the top predictors are the ones you can change. Fitness and strength respond to training at almost any age.
Why most of the numbers you chase do not matter much
Walk into any longevity clinic and you can leave with a printout of a hundred values. Very few of them carry real predictive weight. A number is only useful if people with worse values reliably die sooner, and if changing the value changes the outcome. By that test, most of the panel is noise. A handful of measures clear the bar decisively, and the strongest of them are not blood tests at all.
This matters for your time and money. Chasing a dozen minor markers can feel productive while distracting you from the two or three things that actually move the odds. The rest of this guide is that short list, in rough order of how much the evidence backs each one.
A marker that predicts longevity is not the same as a cause. Fitness predicts a long life partly because it reflects a healthy heart, active muscle, and years of movement. The value in tracking it is that it is measurable, it responds to what you do, and it flags risk early enough to act on.
VO2 max: the strongest single predictor
VO2 max is the maximum amount of oxygen your body can use during hard exercise. It reflects how well your heart, lungs, blood, and muscles work together, which is why it captures so much about your overall condition in one number. Of everything on this list, cardiorespiratory fitness has the strongest link to living longer.
A large study followed 122,000 adults who did treadmill tests and tracked who died over the following years. Higher fitness lined up with lower death rates across the board, and, strikingly, there was no point where more fitness stopped helping. Even the most fit group had lower mortality than the merely fit. The gap between low and high fitness was larger than the gap linked to smoking, diabetes, or high blood pressure in that dataset.
You do not need a lab to work on it. A rough VO2 max estimate comes from many smartwatches, or from a timed effort like how fast you can cover a set distance. The number is useful mainly as a direction of travel. If your fitness is climbing year over year through your forties and fifties, you are moving the marker that matters most.
Grip strength: the cheapest test on the list
Grip strength sounds trivial, but it is one of the best low-cost windows into how you are ageing. Squeezing a handheld gauge takes ten seconds, and the result stands in for total-body strength and muscle health reasonably well. In a study spanning 17 countries, grip strength predicted death better than blood pressure did.
In that research, each **5 kilogram** reduction in grip strength was linked to roughly a **16 percent** higher risk of dying from any cause, along with higher cardiovascular risk. Grip itself is not magic. It is a marker of the muscle and strength you carry, and losing that strength with age tracks with frailty, falls, and a shorter life.
Grip is trainable, and so is the strength it stands for. Regular resistance work, carries, deadlifts, rows, and simply hanging from a bar, builds the strength this test measures. The point is not a strong hand for its own sake. It is the muscle across your whole body that a strong grip reflects.
How fast you walk
Usual walking speed is such a good predictor in older adults that some clinicians call it the sixth vital sign. It bundles together heart function, leg strength, balance, coordination, and nervous-system health into one easy measurement: how far you cover in a set time at a comfortable pace.
A pooled analysis of nearly 34,500 older adults found that walking speed predicted survival at every age, and especially after 75. Among people the same age and sex, faster walkers had dramatically better ten-year survival than slower ones. A common rough marker of concern is a usual pace below about 0.8 metres per second, roughly the speed you need to cross at many pedestrian lights.

None of these needs a clinic. You can time your own walking pace, count how many times you can stand from a chair in thirty seconds, and retest every few months. Because they respond to training, a bad first result is an invitation, not a verdict.
The blood markers actually worth watching
Blood tests do matter, but far fewer of them than the panels on sale suggest. The ones with the strongest links to how long you live cluster around four systems: how you handle sugar, your blood pressure, your blood fats, and your background level of inflammation. Get those into good ranges and you have covered most of what a blood draw can tell you about longevity.
- Blood sugar control. A raised HbA1c, which reflects your average blood sugar over about three months, flags rising diabetes risk. The widely used threshold for prediabetes is 5.7 percent.
- Blood pressure. Persistently high pressure quietly damages arteries for years. A common target for healthy adults is around 120/80 or below, confirmed with repeat readings.
- Blood lipids. Rather than total cholesterol alone, the number of harmful particles, often measured as ApoB or non-HDL cholesterol, tracks cardiovascular risk more closely.
- Inflammation. A sensitive test called hs-CRP picks up low-grade, long-running inflammation that is linked to heart disease and other age-related conditions.
Long boutique panels of hormones, heavy-metal screens, and dozens of micronutrients rarely change what a healthy person should do. If a result would not alter your training, your diet, or a conversation with your doctor, paying to measure it mostly buys anxiety. Start with the four systems above and a talk with a clinician about what your history warrants.
What to actually do with all this
The encouraging part is that the strongest predictors are the ones you have the most control over. You cannot change your age or your genes, but fitness, strength, and mobility all respond to training well into later life. The blood markers respond too, often to the same habits.
- Build aerobic fitness first. It is the top predictor. Aim for at least 150 minutes of moderate activity a week, with some harder efforts mixed in, and treat rising fitness as your headline number.
- Lift and carry things. Two or three strength sessions a week protect the muscle behind your grip strength, your walking speed, and your independence later on.
- Measure a few functional markers. Time your walking pace, count your sit-to-stands, and, if you can, estimate your VO2 max. Retest every few months to see the trend.
- Test the four blood systems, not the whole catalogue. Blood sugar, blood pressure, lipids, and inflammation cover most of what a draw can tell you. Discuss anything unusual with your doctor rather than a lab website.
- Watch trends, not single readings. One value on one day means little. The direction over years is the real signal, and it is the one your habits actually shape.
If you only had time to work on one thing, it would be your fitness, and if you had time for two, add strength. The markers that predict a long life the best are also the ones most in your hands.
