Ask around and you will hear the same two things about skull crushers: they build serious triceps, and they wreck your elbows. Both are true, and both come from the same source. The exercise loads the triceps hard in a stretched position, which is great for growth, but that stretch also puts real stress through the elbow, and the way most people set up and load the movement piles that stress on until the joint complains. The good news is that elbow pain from skull crushers is usually a form and loading problem, not an inevitable price. Fix a few things and you can keep the growth and lose the ache.

Why skull crushers are worth the trouble
A skull crusher is a lying triceps extension: you lie on a bench, hold a weight above you, and lower it toward your head by bending at the elbows, then extend back up. What makes it valuable is that it loads the triceps in a lengthened, stretched position at the bottom, and the stretched position is where a muscle tends to do its most growth-producing work.
It is especially good for the long head, the largest of the three triceps heads, which runs from the shoulder blade down the back of the arm. Because the long head crosses the shoulder, angling your arms slightly overhead loads it much more than a pushdown ever can. That combination, a big muscle trained in a deep stretch, is exactly why skull crushers build arms. It is also exactly why they can bother the elbow, because the stretched, loaded position puts stress through the joint and its tendons.
The stretch that makes skull crushers effective is the same stretch that stresses the elbow. You are not trying to remove that stress, you are trying to keep it in a range the joint can handle while you build the muscle. That is what the rest of this guide is about.
Where the elbow pain comes from
Skull crusher elbow pain almost always traces back to a handful of avoidable causes. It is rarely the exercise being fundamentally bad for you.
- Too much weight. Ego loading forces you to bounce out of the bottom and strains the tendons rather than the muscle.
- A straight bar. Fixing your wrists in a straight-bar grip drives awkward stress into the wrist and elbow. An EZ bar or dumbbells let the joints sit more naturally.
- Lowering to the wrong spot with the wrong arm angle, so the joint takes the load at its most vulnerable point.
- Bouncing at the bottom, jerking out of the stretch instead of controlling it, which spikes the force through the tendon.
- Training triceps too often, so the muscle recovers but the slower-healing tendons and connective tissue never catch up.
Your triceps might feel ready to go again in a day or two, but tendons and connective tissue heal more slowly. Give the joint time, and treat elbow ache as a signal to back off, not to push through.
How to do skull crushers without the ache
Here is the setup that keeps the growth and protects the joint. The changes are small, and together they make a large difference.
- Use an EZ bar or dumbbells. The angled grip of an EZ bar, or the neutral grip of dumbbells, lets your wrists and elbows sit in a more comfortable line. A straight bar forces a fixed position that many elbows dislike. This one change fixes a lot of pain on its own.
- Angle your upper arms slightly back. Instead of holding your upper arms straight up so the weight travels to your forehead, tilt them a little toward your head so the movement is more of an extension behind you. This loads the long head better and shifts the movement into a friendlier position for the elbow.
- Lower under control to a full stretch. Bend at the elbows and lower the weight slowly, feeling the triceps stretch. Take it toward the top of your head or just behind it, wherever your setup and comfort allow, without any bouncing.
- Extend without locking hard. Press back up by straightening the elbows and squeezing the triceps. You do not need to slam into a violent lockout. A smooth, controlled extension keeps tension on the muscle and spares the joint.
- Keep the reps smooth, not jerky. Every rep should look the same: controlled down, controlled up, no momentum. If you have to heave or bounce out of the bottom, the weight is too heavy for a healthy skull crusher.
Try an overhead cable extension or a dumbbell overhead extension instead. They train the long head in the same stretched position but keep constant tension and often feel gentler on the elbow than a barbell skull crusher. You lose nothing by swapping to the version your joints tolerate.
Loading and programming for healthy elbows
Form protects the joint in each rep. Sensible loading and programming protect it across weeks, which is where most elbow trouble actually builds up.

Warm up the elbows before you load them. A couple of light sets of pushdowns or extensions gets blood into the joint and tendons before you ask them to handle a stretch under load. It costs two minutes and saves a lot of grief.
Finally, treat elbow pain as information. A little muscular fatigue is normal, but a sharp or lingering ache in the joint means back off: lighter weight, fewer sets, or swap to the overhead cable version for a while. Pushing through joint pain is how a manageable niggle becomes a lasting problem. If pain persists despite sensible changes, it is worth talking to a physiotherapist or doctor rather than guessing.
