The bench press is one of the most common causes of shoulder pain in the gym, and it is almost never the exercise itself that is the problem. It is how the shoulder is set up and how the bar travels. Your shoulder is a shallow, mobile joint held together mostly by soft tissue, and a wide grip with flared elbows puts the front of it in the worst possible position under a heavy load. The good news is that the fixes are simple, they cost you almost no weight on the bar, and most people feel the difference in a single session.
The short version
- Most bench shoulder pain comes from a wide grip and flared elbows, which crowd the front of the shoulder joint under load.
- Bring your grip in to about 1.5 shoulder widths or less and tuck your elbows to roughly 45 to 70 degrees from your body.
- Set your shoulder blades **back and down** and keep them pinned there for the whole set. This is the single biggest fix.
- Touch the bar to your lower chest, not your neck, so your forearms stay stacked and your elbows stay under the bar.
- Sharp or pinching pain in the front of the shoulder is a signal to change your setup, not to push through it.
Why the bench press hurts your shoulder
Your shoulder is built for range of motion, not stability. The ball of your upper arm sits against a shallow socket, held in place by the rotator cuff and the surrounding tissue rather than by deep bone. That trade lets you reach in almost any direction, but it also means that under a heavy bar, small changes in position make a large difference to how much stress lands on the joint.
The classic painful bench looks like this: a wide grip, elbows flared straight out to the sides, and the bar tracking up toward the neck. In that position your upper arm is pulled away from your body at close to 90 degrees, which crowds the space at the front and top of the shoulder and puts the tendons there under a pinch. Do that for enough reps, over enough weeks, and the front of the shoulder starts to ache, then to bark on your warm up sets.
The pain most people feel is at the front of the shoulder, where the rotator cuff tendons and the top of the biceps tendon pass through a tight space. A flared, wide setup narrows that space under load. A tucked, narrower setup opens it.
Grip width: narrower than you think
Grip width is the first thing to change, because it sets everything downstream. A very wide grip feels powerful and shortens the distance the bar travels, which is why competitive lifters use it. But a biomechanical modelling study of bench technique found that widening the grip from about 1.5 to 2 shoulder widths significantly increased the total force and compression at the shoulder joint, while keeping the grip at 1.5 shoulder widths or less lowered that loading.
In plain terms: pulling your hands in a little takes stress off the joint. The same study found that a narrower grip also reduced compression at the top of the shoulder, the spot linked to wear at the end of the collarbone. Bringing the hands in is a trade almost everyone with a cranky shoulder should take.
- Find your baseline. Grip the bar so that at the bottom of the press your forearms are roughly vertical when viewed from the front. If your hands are much wider than that, you are past the useful zone.
- Aim for 1.5 shoulder widths or narrower. Measure it roughly by eye: the distance between your hands is about one and a half times the distance between the bony points of your shoulders, or less.
- Let your chest do more. A slightly narrower grip brings the triceps into the lift more and reduces the extreme stretch on the front of the shoulder at the bottom.
Elbow angle: tuck, do not flare
If grip width sets your hands, elbow angle decides where your upper arms go. The painful version is a full flare, where the elbows point straight out to the sides and the upper arm sits at close to 90 degrees from the torso. The safer version tucks the elbows in, so the upper arm makes an angle of roughly 45 to 70 degrees from the body when the bar touches your chest.
Tucking does two things. It moves your upper arm out of the crowded overhead position and into a range where the shoulder has more room, and it keeps the bar lower on your chest so your forearms stay stacked under the bar. You do not need to tuck so hard that your elbows are glued to your ribs, which robs the chest of leverage. Somewhere between a full flare and a full tuck is the sweet spot for most people.
Think of pressing the bar in a slight arc from your lower chest up toward your face, rather than straight up from your collarbone. That path naturally tucks your elbows and keeps your shoulders back.
The setup that matters most: your shoulder blades
This is the fix that changes the most for the least effort. Before you unrack the bar, pull your shoulder blades back and down, as if you were tucking them into your back pockets, and pin them there. This gives the ball of your shoulder a stable shelf to press from, instead of letting it roll forward toward the tender front of the joint on every rep.
The biomechanical evidence backs this up. Setting the shoulder blades in a retracted, depressed position reduced the shear and compression forces at the shoulder during the middle of the press, spreading the load more evenly across the joint. When people let their shoulders round forward and travel with the bar, that protection disappears and the front of the joint takes the hit.
- Set before you unrack. Lie down, plant your feet, and pull your shoulder blades back and down into the bench. Feel your upper back tighten and your chest lift slightly.
- Keep them pinned. Hold that position through the entire set. Your shoulders should not shrug up toward your ears or roll forward as you press.
- Use your legs. Drive your feet into the floor to help hold your upper back tight. A stable base makes it far easier to keep the shoulder blades set.
Letting your shoulders round forward at the bottom of the rep, or shrugging them up toward your ears as you press. Both push the ball of the shoulder into the front of the joint. If your shoulders creep, lighten the load until you can hold them back and down for every rep.
Bar path and touch point
Where the bar touches your chest is tied directly to your elbow angle. Touch it too high, up near the collarbone or neck, and your elbows have to flare and your upper arms climb into the painful position. Touch it lower, around the base of the sternum or just below the nipple line, and your elbows stay tucked and your forearms stay stacked vertically under the bar.

None of this means the bar path is a rigid straight line. A slight diagonal, from a lower touch point up and back toward your shoulders at the top, is normal and healthy. What you are avoiding is the vertical column of force pressed straight up from a high touch point with flared elbows, which is the setup that most reliably grinds the joint.
When it already hurts
If your shoulder is already sore, fixing your setup is the first move, but you may also need to back off for a while. Sharp or pinching pain at the front of the shoulder is a signal, not a challenge to push through. Working into that pain repeatedly is how a minor irritation becomes a stubborn one.
- Drop the load and fix the setup. Lighten the bar, apply the narrower grip, the tuck, and the pinned shoulder blades, and rebuild from there.
- Swap to pain free variations for a bit. A slight incline, a neutral grip dumbbell press, or a floor press often let you keep training the chest while the joint calms down.
- Build the back of the shoulder. Rows, face pulls, and rear delt work balance out all the pressing and help hold the joint in a healthier position over time.
- Get it looked at if it lingers. Pain that does not settle after a few weeks of sensible changes is worth a visit to a physiotherapist or doctor. Persistent shoulder pain is not something to just train around forever.
You have pain that wakes you at night, weakness or a sudden loss of strength, a feeling that the shoulder is slipping or catching, or pain that keeps getting worse despite backing off. These deserve a proper assessment rather than a form tweak.
