It is one of the most repeated claims in nutrition: diet drinks and sweeteners fool your body into expecting sugar, and when the sugar never arrives, you end up craving more of it and eating extra later. It sounds mechanically obvious, which is part of why it spread so far. The problem is that when researchers actually test it, feeding people sweeteners and measuring what they eat next, the effect mostly fails to show up. The real picture is more reassuring, and a bit more nuanced, than either the panic or the marketing suggests.

The theory, and why it sounds right
The argument goes like this. When you taste something sweet, your body prepares for incoming sugar: this is sometimes called the cephalic phase response. With a sweetener there are no calories to follow, so the expectation is unmet. The theory says this mismatch, repeated over time, confuses appetite regulation, leaves you unsatisfied, and drives you to seek out real sugar to close the gap. Some versions add that sweeteners keep your taste for sweetness cranked up, making everything else taste bland.
It is a genuinely plausible story, and pieces of it have shown up in laboratory and animal work. That is why it gets repeated by people who are not being careless. But plausible is not the same as proven, and appetite is famously good at compensating in ways simple theories miss. The honest test is not whether the mechanism could work, but whether people who use sweeteners actually end up craving and eating more.
Not "could sweeteners trick appetite in theory" but "do people given sweeteners actually eat more sugar and gain weight". That is a question you can test directly, and it has been tested.
What the trials actually found
A large systematic review pulled together the human evidence, including randomised controlled trials, the kind of study that can show cause and effect rather than just correlation. The pattern ran opposite to the craving theory. When people had a low-calorie sweetener before a meal instead of sugar, they ate fewer total calories at that meal, on average around 94 fewer calories. Over longer trials lasting weeks to months, swapping sugar for sweeteners was linked to modestly lower body weight, on the order of a 1.3 kilogram difference, not the weight gain the theory predicts.
If sweeteners reliably drove people to crave and consume more sugar, you would expect the opposite result in these trials. Instead, on average, replacing sugar with a sweetener reduced intake. That does not make sweeteners a health tonic, and the effects are modest, but it directly undercuts the idea that they trigger a compensating sugar binge in the typical person.
Observational studies sometimes link diet drinks to higher weight. That is usually reverse causation: people who are already gaining weight or managing diabetes switch to diet versions. It does not show the sweetener caused the weight, which is why the randomised trials matter more here.
What really drives sugar cravings
If sweeteners are not the main culprit, what is? Cravings for sweet food are driven far more by ordinary, well-understood things, and this is good news because each one is something you can influence.
- Habit and cues. The biscuit with tea, the pudding after dinner. Much of what feels like a craving is a routine firing on schedule, with or without sweeteners in the picture.
- Genuine hunger. Under-eating, skipping meals, or skimping on protein and fibre leaves you hungry, and sweet food is the fastest fix your brain reaches for.
- Restriction. Banning sugar tends to increase how much you think about it and want it. The rebound is a bigger driver of cravings than any sweetener.
- Sleep and stress. Both reliably push up appetite and the pull toward sweet, high-reward food, independent of what you drink.
Notice that none of these are on the label of a diet drink. Blaming the sweetener can be a comfortable distraction from the meal you skipped or the four hours of sleep you lost. If your cravings are relentless, these are the levers worth checking first.
The honest caveat about sweetness itself
There is a fair point buried inside the craving theory, even if the strong version fails. Sweeteners do keep you accustomed to intense sweetness. If your day is a stream of sweet coffee, sweet drinks, and sweet snacks, all zero calorie, your palate stays calibrated to expect that hit, and plain or savoury food can feel dull by comparison. This is about your taste preference over time, not a chemical craving trap, but it is real and worth respecting.
That is why the smartest use of sweeteners is as a bridge, not a destination. They are excellent for cutting a large chunk of sugar out of something you already drink or eat, especially in the early weeks of changing a habit. Over time, gradually dialling down overall sweetness, sugar and sweetener alike, tends to reset your palate so you need less of it to feel satisfied.
Use sweeteners to replace sugar in specific things, like a fizzy drink or your coffee, rather than to add sweetness everywhere. The goal is less sweetness overall, not just sugar-free sweetness on tap.
The bottom line
The claim that sweeteners make you crave more sugar is intuitive, widely repeated, and mostly not supported by the controlled human evidence. On average, swapping sugar for a sweetener leads people to eat slightly less, not more. Cravings come mainly from habit, hunger, restriction, and poor sleep, none of which a sweetener causes.
The sensible position sits between the two camps. Sweeteners are a genuinely useful tool for cutting sugar, particularly in drinks, and they are not the appetite trap the myth makes them out to be. They are also not a free pass to bathe your palate in sweetness all day. Use them to reduce sugar, aim to need less sweetness over time, and put your real effort into eating enough and sleeping enough. That is where the cravings actually live.
